Our Home Health and Hospice partners provide a variety of skilled nursing services.  These include: 

  • Wound Care
  • IV Infusions/Injections
  • Diabetic Care and Teaching
  • Medication Education and Management
  • Enteral Feeding
  • Ostomy/Catheter Care
  • Post Surgical Care 
  • PT/INR

Our Home Health and Hospice partners have Doctors of Physical Therapy and 

Physical Therapists that provide services such as:  

  • Balance and Gait Training
  • Mobility and Flexibility Training 
  • Strengthening Exercises
  • Transfer Techniques

These services help our patients limit falls by showing them how to safely get out of bed, step out of the tub, a

These services help our patients limit falls by showing them how to safely get out of bed, step out of the tub, and get off the toilet.  CSI ensures that therapy is administered, patient sees progress from therapy, and if needed, is receiving additional visits.  We help make sure patients with chronic diseases like Parkinson's, ALS, and Alzheimer's are able to access to physical therapy throughout the year.

at-home rehabilitation.  


Occupational Therapy services include:

CSI understands that while this service is not paid for by Medicare Insurance, unlike the services above, our Home Health and Hospice partners provide this discipline to patients that we service.  We value our collaboration with dietitians because of their ability to convey to patients the importance of what they eat every day, and how to control portion sizes.  We have seen patients benefit greatly from this service, with improvements in disease management.  Our diabetic patients especially benefit from learning about dietary strategies that help control their chronic disease.

By consulting with our doctors, patient families, and health care communities, we can help seniors that are considered fall risks.

Patients considered fall risks include: 

  • Those who have trouble walking, even short distances
  • Those with weakness in their legs and/or knees
  • Those who already fall frequently

CHF/COPD Program

CHF/COPD Program

CSI, along with our Home Health and Hospices partners, provide help with these two serious conditions: CHF and COPD. 


CHF (Congestive Heart Failure): We monitor heart rate, oxygen levels, and blood pressure. We help patients understand their eating habits and make healthier choices, while we have nurses help with medication management.

CSI, along with our Home Health and Hospices partners, provide help with these two serious conditions: CHF and COPD.

COPD (Chronic Obstructive Pulmonary Disease): Our nursing and physical therapy staff help with exercise, breathing techniques, nutrition, relaxation, and medication management. 

How to pay for our services:

The services provided by our Home Health and Hospice partners are covered by your Medicare Health Insurance, granted it is your primary insurance.  Medicare benefits cover all these services with no co-pays or no out of pocket costs to you.
